COVID-19 Safety Protocols
Assumption of Risk and Waiver of Liability Relating to Covid-19:
- We are and continue following the guidelines set up for the School District of Palm Beach County in regards to Covid-19.
- Our lobby will continue to be closed and pick up for the first couple of hour long classes (4pm & 5pm) will be from the doors at the front of our studio along Dixie Highway.
- Masks are up to the individual families.
- Families are asked to continue to keep their student home if they are sick and inform FDC should their student test positive for Covid-19.
- The World Health Association has declared the novel Coronavirus (Covid-19) a worldwide pandemic. due to its capacity to transmit from person-to-person through respiratory droplets, the federal, state and local governments have set recommendations, guidelines and some prohibitions which Florida Dance Conservatory (FDC) adheres to comply. (examples listed but not limited to below)
- Closed lobby to all except staff and students.
- Face masks required for students, to be worn through out the class.
- Limited class size.
- Temperature checks and hand sanitize upon entry.
- Florida Dance Conservatory has put into place preventative measures to reduce the spread of Covid-19, however, FDC cannot guarantee that you or your children will not become infected with Covid-19.
- In consideration of your child's participation in the foregoing, the undersigned acknowledge and agree to the following:
- I am aware of the existence of the risk of exposure to the Covid-19 virus on my physical appearance to the venue and my participation in the activity of FDC.
- I have been, nor any member(s) of my household, diagnosed to be infected of Covid-19 virus within the last 30 days.
- I have not, nor any member(s) of my household, experienced symptoms of fever, fatigue, difficulty in breathing, or dry cough or exhibiting any other symptoms relating to Covid-19 within the last 14 days.
- In consideration of students, staff and other families, I will be responsible and relay any change, possible exposure or diagnosis of myself, my child(ren) or any family member to FDC so that families may be notified to the possibility of exposure (contact tracing), and I will remove my child from classes for a minimum of 14 day quarantine should the need arise. FDC will not disclose personal information as related to any need to notify students and families of possible exposure.
- Following the pronouncements above I hereby declare the following:
- I fully recognize that there may be a risk of exposure or of contacting Covid-19 while participating at FDC.
- With full knowledge of the risks involved, I hereby release, waive, discharge FDC, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions and causes of action whatsoever, directly or indirectly arising out of or related to any los, damage, injury, or death that may be sustained by my child/family related to Covid-19 while participating in any activity while in, on, or around the premises or while using the facilities that may lead to unintentional exposure or harm due to Covid-19.
- I agree to indemnify, defend, and hold harmless FDC from and against any and all costs, expensed, damages, lawsuits, and/or liabilities or claims arising whether directly or indirectly from or related to any and all claims made by or against any of the released party due to injury, loss, or death from or related to Covid-19
- By acknowledging below I state that I have read the foregoing Liability Release Waiver and understand its contents. That I have been sufficiently informed of the risks involved in participation and give my voluntary consent as my own free act and deed.
- This waiver will remain effective until laws and mandates relevant to Covid-19 are lifted.
